Sonde bimodale TEP/IO : application en cancérologie

Résumé

Medical imaging is spreading more and more into a bimodal system. Considerable research has been conducted in this field of hybrid contrast agents over the last years. Therefore, Positron-Emission Tomography (PET), a sensitive method, can allow the identification and localization of a tumor and the therapy monitoring, while fluorescence can help during surgery. A probe with these two kinds of medical imaging would make possible the entire patient support, from the diagnosis of the cancer to the treatment by surgery and the therapy monitoring. This project consists in the development of a bimodal probe PET/OI, based on the functionalization of a cyclotriveratrylene (CTV) used as a template. This macromolecule presents a bowl shape with three “arms”. These three parts are functionalized with a cRDG, a peptide able to recognize the activity of αvβ3 integrins, an arylsilane, allowing the introduction of a 18F during the last step of the synthesis for PET imaging and a fluorophore (as first intention, a benzothiadiazole - BTD). The synthesis of the different synthons as well as the coupling with the platform by “click” chemistry reactions leading to the obtention of the first desired bimodal probe are presented in this manuscript. The fluorescence of some intermediate products and of the final molecule is measured. Some preliminary biological analyses are conducted, like cytotoxicity test, the radiance, and the in vitro fluorescence as well as the cold fluorination and radiosynthesis of the probe. The second part of this thesis describes some ways of improvement for the probe, notably concerning the fluorescence properties. The synthesis of other fluorophores is presented and the coupling with the one that has presented the most favorable properties (a BODIPY) is described allowing to consider the obtention of a very promising second-generation probe for in vivo applications.
La nécessité de détecter de manière encore plus efficace et surtout plus spécifique les tumeurs ou d’autres maladies a fait évoluer les techniques d’imagerie médicale vers la bimodalité, voire la multimodalité. Durant ces dernières années, de nombreuses recherches ont été menées dans le but de concevoir des agents de contraste hybrides. La Tomographie par Emission de Positon (TEP) est une méthode sensible qui permet l’identification et la localisation d’une tumeur ainsi qu’un suivi thérapeutique. L’imagerie optique (via la fluorescence) permet de guider le chirurgien pendant la tumorectomie. Une sonde portant ces deux types d’imagerie pourrait rendre possible la prise en charge complète d’un patient, du diagnostic d’un cancer à son traitement par chirurgie, puis son suivi post opératoire. Ce projet consiste donc à développer une sonde bimodale TEP/IO basée sur la fonctionnalisation d’un cyclotrivératrilène (CTV) utilisé comme plateforme. En effet, le CTV présente une forme de bol avec trois « bras » fonctionnalisables. Il a ainsi été envisagé d’introduire sur ces « bras » un cRGD, permettant la reconnaissance spécifique des intégrines αvβ3, un arylsilane permettant d’introduire en dernière étape de synthèse un 18F nécessaire à la TEP, ainsi qu’un fluorophore (en premier lieu un benzothiadiazole - BTD). La synthèse de ces différentes molécules ainsi que leur couplage sur la plateforme conduisant à la première sonde bimodale désirée sont présentés. Les propriétés de fluorescence de certains intermédiaires et de la molécule finale sont évaluées. Des analyses biologiques préliminaires, comme la cytotoxicité, la radiance et la fluorescence in vitro ainsi que la fluoration à froid et la radiosynthèse sont exposées. Dans une seconde partie, des voies d’amélioration de cette première sonde sont explorées, notamment pour optimiser les propriétés de fluorescence. Un BODIPY est alors utilisé pour remplacer le BTD permettant d’envisager l’obtention d’une sonde de deuxième génération prometteuse pour des applications in vivo.
