Effets différentiels d'un entraînement neurofeedback électroencéphalographique auprès de personnes âgées avec MCI

Le neurofeedback électroencéphalographique ( NF EEG) est une technique d'apprentissage de l'auto-régulation de l'activité électrique cérébrale. Un patient reçoit des informations sur son activité cérébrale en temps réel qu'il peut modifier grâce à la mobilisation de capacités attentionnelles et d'un certain état de vigilance. Les études portant sur les effets du NF sur les troubles cognitifs liés au vieillissement sont récentes. Le champ d'application du NF en gérontologie se développe rapidement auprès des personnes avec troubles cognitifs légers (ou Mild Cognitive Impairment-MCI) et avec la Maladie d'Alzheimer. Les protocoles NF et les paramètres d'application se précisent grâce aux recommandations d'études rigoureuses. Néanmoins, ils nécessitent encore des investigations plus poussées pour optimiser l'efficacité de la technique dans l'amélioration des capacités cognitives auprès des sujets âgées souffrant de déficiences cognitives. Ce travail de thèse vise à définir les mécanismes et les fondements théoriques qui sous-tendent le NF EEG mais aussi les concepts neuropsychologiques, physiologiques et électroencéphalographiques dans les troubles cognitifs légers et du stade prodromal de la maladie d'Alzheimer. Quatre études ont été menées à partir d'un travail expérimental réalisé auprès de vingt participants avec MCI ayant suivi un entraînement neurofeedback visant à augmenter l'amplitude du rythme sensorimoteur (12-15Hz) et à diminuer l'amplitude des ondes theta (4-8Hz) au-dessus du cortex central (Cz). La première étude ayant évalué la faisabilité du programme NF auprès de sujets âgés avec MCI a permis de cibler les limites et les améliorations possibles de la technique du NF pour une meilleure acceptabilité et adhésion par cette population. Les résultats de la seconde étude ont montré des améliorations significatives des performances cognitives et psycho-affectives après les entraînements NF. L'activité des bandes de puissance theta et alpha avait également augmenté significativement mais sans qu'il ne soit avéré de corrélation avec les bénéfices cognitifs. L'analyse à un mois de suivi a montré l'absence de maintien des progrès cognitifs mais la persistance de changements EEG. La troisième étude a permis d'identifier l'absence d'effet du NF chez les patients Pdr-MA contrairement aux personnes avec MCI qui ont bénéficié d'améliorations des performances cognitives et d'activités EEG plus intenses. La quatrième étude a permis de noter un nombre élevé de pics d'amplitude d'ondes lentes pendant l'entraînement des jeux et de supposer le lien probable avec les épisodes de somnolence survenus chez des participants. La nécessité de confirmer nos résultats dans un essai randomisé et contrôlé avec un échantillon de participants nettement plus conséquent nous a conduits à élaborer le protocole d'une prochaine étude de recherche. L'objectif consistera à évaluer et comparer les effets de deux programmes d'entraînement NF et à trois mois de suivi.
Electroencephalographic neurofeedback (NF EEG) is a technique for learning self-regulation of cerebral activity. A patient receives information about her/his brain activity in real time that she/he can modify through a mobilization of attentive abilities and a certain state of vigilance. The effects on behavioral, cognitive and thymic functions by neurofeedback training are studied in research work in neurology, psychiatry, mathematics and technological sciences. This stimulation tool seems to be a promising technique in the mitigation and prevention of certain conditions such as children and adolescents with attention deficit/hyperactivity disorder (ADHD). Studies of the effects of NF on aging-related cognitive disorders are recent. The scope of NF in gerontology has been growing rapidly among people with mild cognitive impairment (MCI) and Alzheimer's disease. NF protocols and application parameters are more precise thanks to rigorous study recommendations. Nevertheless, they still require further investigation to optimize the effectiveness of the technique in improving cognitive abilities in elderly subjects with MCI. This thesis aims to define the mechanisms and theoretical underpinnings that underlie the NF EEG as well as the neuropsychological, physiological and electroencephalographic concepts in MCI, prodromal stage of Alzheimer's disease and Alzheimer's disease. The second part of this thesis develops the different studies carried out from an experimental work with twenty participants with MCI that followed a neurofeedback training aiming to increase the amplitude of the sensorimotor rhythm (12-15Hz) and to reduce the amplitude of theta waves (4-8Hz) above the central cortex (Cz). The first study assessed the feasibility of a NF program in elderly subjects with MCI. The results identified the limits and the possible improvements of the NF technique for a better acceptability and adhesion by this population. The second study aimed to explore the effects of the NF SMR/theta training on the cognitive performance and cerebral electrical activity of these twenty subjects. The results showed significant improvements in cognitive and psycho-emotional performance after NF training. The activity of the theta and alpha powers had also increased significantly but without any correlation with cognitive benefits. The one-month follow-up analysis showed the lack of maintenance of cognitive progress but the persistence of EEG changes. The third study explored the differential effects of NF training between fifteen patients with MCI and five subjects at the prodromal stage of AD (Pdr-AD). The results identified the lack of effects of NF in Pdr-AD patients as opposed to people with MCI who showed improvements in cognitive performance and more intense EEG activities. The fourth study aimed to observe the effects of 11 NF interfaces on the EEG activity with 5 participants. The results noted a high number of slow wave amplitude peaks during the training of all these games. We assumed the probably link with the drowsiness episodes that occurred in participants. The need to confirm our results in a randomized controlled trial with a much larger sample of participants led us to develop a new research protocol. The objective will be to evaluate and compare the effects of two NF training programs and three months of follow-up.
