Herpes simplex virus (HSV) resistance to acyclovir (ACV) can be due to mutations occuring either in one or two genes implicated in mechanism of action of the drug, the thymidine kinase gene (TK, 95% of the cases) or the DNA polymerase gene. TK gene reveals a high degree of polymorphism and distinction between mutations related to ACV resistance or to TK polymorphism can be difficult. The study developed herein allows this distinction by using a site-directed mutagenesis method, a protein production by a reticulocytes lysat and an enzymatic test of the mutated proteins. This development of the data base of the TK mutations related to ACV resistance is necessary to genotypic test. Herein, we present a rapid PCR-based assay, performed directly on clinical samples, based on the amplification of the entire TK gene followed by PCR products sequencing. It is the first time that a genotyping assay is described to detect HSV resistance to ACV without virus culture. With this technique, we should be able to detect HSV resistance to ACV up to 90% of the cases in 24 hours
