Expression du gène de résistance multidrogue MDR1 chez l'homme : dans les hépatocytes au cours de la carcinogénèse hépatique, dans les lymphocytes chez les transplantés hépatiques

La P-glycoprotéine (P-gp), produit du gène de résistance multidrogue MDR1, est une protéine membranaire appartenant à la famille des protéines ABC ("ATP-Binding Cassette"), qui entraîne l'efflux d'un grand nombre de molécules hors de la cellule. La première partie de l'étude a mis en évidence une surexpression de la P-gp, au niveau de la membrane des hépatocytes, dans le carcinome hépatocellulaire (CHC) chez l'homme, par rapport au foie péritumoral adjacent. Cette surexpression a été démontrée dans 65% à 85% des cas, par immunohistochimie, à l'aide des anticorps monoclonaux anti-P-gp UIC2 et C-494. Ces données étaient confirmées au niveau de I'ARNm, par RT PCR, suggérant une régulation transcriptionnelle. Les expressions de I'ARNm MDR1 et de la P-gp étaient plus élevées dans le foie péritumoral cirrhotique ou dans la cirrhose non tumorale que dans le foie péritumoral non cirrhotique ou dans le foie sain, suggérant que la surexpression de la P-gp pourrait constituer un marqueur prétumoral. Parallèlement, une surexpression du gène MDR1 était montrée in vitro dans les lignées d'hépatomes humains (HepG2, Hep3B, PLC/PRF5), par rapport aux hépatocytes humains frais et normaux en culture primaire. Dans une seconde partie, un suivi de l'expression de I'ARNm MDR1 et de la P-gp était réalisé dans les lymphocytes périphériques des transplantés hépatiques, au cours du traitement immunosuppresseur. L'expression de la P-gp, mesurée par cytométrie en flux, augmentait sensiblement après transplantation. Les variations de l'expression de la P-gp ne permettaient pas de prédire le rejet. Expression et fonction de la pompe étaient également légèrement augmentées dans des lymphocytes de donneurs sains soumis in vitro à un traitement immunosuppresseur. En conclusion, cette thèse a investigué l'expression du gène MDR1 dans les hépatocytes au cours de la carcinogénèse hépatique et dans les lymphocytes après transplantation hépatique. Elle met en évidence le rôle potentiellement important de la Pgp exprimée dans ces deux types cellulaires, et montre la possible utilisation de modèles in vitro pour une meilleure compréhension du phénomène MOR.
P-glycoprotein (P-gp), the multidrug resistance MDR1 gene product, is a membrane protein which belongs to the ABC (ATP-Binding Cassette) protein family and is associated with the efflux of a large variety of molecules out of the cell. The first part of this study has shown an overexpression of P-gp, on the membrane of hepatocytes, in hepatocellular carcinoma (HCC) in human, in comparison to adjacent peritumoral liver tissue. This overexpression occurred in 65% to 85% of the cases and was assessed by immunohistochemistry, using UIC2 and C-494 anti-P-gp monoclonal antibodies. These data were confirmed at the mRNA level, by RT-PCR, implicating transcriptional regulation. MDR1 mANA and P-gp expression were also higher in cirrhotic peritumoral liver or in non tumoral cirrhosis than in non cirrhotic peritumoral liver or in normal liver, suggesting that P-gp overexpression might constitute a pretumoral marker. Ln parallel, overexpression of the MDR1 gene was seen in hepatoma cell lines (HepG2, Hep3B, and PLC/PRF5), in comparison with its expression in human fresh, normal hepatocytes in primary culture. Ln the second part, MDR1 mRNA and P-gp expression were assessed in peripheral blood mononuclear cells of liver transplanted patients, receiving immunosuppressive treatment. P-gp expression, assessed by flow cytometry, increased after transplantation. Variations in P-gp expression did not permit any prediction of rejection. P-gp expression and function were also increased in health volunteer donor lymphocytes treated by immunosuppressant drugs in vitro. Ln conclusion, this thesis has investigated MDR1 gene expression in hepatocyte associated with liver carcinogenesis and in peripheral lymphocytes from liver transplantee patients. The potential role of P-gp in these two cellular types has been considered, and the possibility of using in vitro models for a better understanding of the MOR phenomenon associated with these pathologies has been discussed.
